Primary Care
Telehealth is long distance medical care using computer videoconferencing. It is similar to “Facetime” on your cellular phone. You must be referred to me by your primary care provider (PCP) for consultation and treatment of hepatitis C. Every person should have a PCP.
Immunizations
Hepatitis C is frequently transmitted by injecting heroin or other drugs. A blood transfusion before July 1992 is sometimes the source. Sexual transmission and from mother to baby are very uncommon. Tattoos, especially jailhouse or those done by a friend, can transmit the virus. Outbreaks from endoscopy centers, dental clinics and from hospitals have been documented. Airgun vaccinations have been proven to transmit viral hepatitis B and many military veterans were infected this way. Fist fights, accidents, sharing razors or anything that mixes one persons blood with another will transmit the virus. A substantial number of patients have no risk factor. Having your neck shaved at a barber shop could transmit HCV or HBV.

Endoscopy
Portal hypertension is a complication of cirrhosis. Blood from the intestines carry nutrients to the liver through the portal vein. The pressure in this vein is usually very low. When the liver is hard and nodular the blood cannot flow through it normally. The pressure in the portal vein rises to high levels. This causes the spleen to become congested and enlarged. Enlarged veins in the esophagus and stomach form called varices. Varices should be suspected when the platelet count falls to less than 110,000 or when there is ascites, jaundice or visible veins in the abdominal wall. Since these varices can rupture and bleed, patients with cirrhosis need upper endoscopy. Esophageal band ligation can treat varices and prevent rupture.

Ultrasound
Screening for liver cancer is done with imaging tests typically with an abdominal ultrasound examination. A CT examination with intravenous contrast is often performed if the ultrasound reveals a heterogeneous appearance or if there is excessive fat in the liver. A triple phase contrast examination is needed called hemangioma protocol. Liver cancers will show up as bright spots a few seconds after injecting the intravenous contrast. A few seconds later they contrast washes out and the cancers blend into the rest of the liver. This is why the images must be obtained before, immediately after contrast injection and a few minutes later.
MRI
HCV is the most common cause of liver related deaths in the United States. HCV causes liver injury and fibrosis. This worsens over 20-40 years and progresses to cirrhosis. A cirrhotic liver is hard and nodular. It cannot function normally and causes progressive fatigue. Eventually, patients develop liver failure or liver cancer. About 2% of patients with cirrhosis develop liver cancer each year. About 3% of patients with cirrhosis progress to liver failure each year. Liver cancer is detected on ultrasound, CT or MRI scans of the liver. Liver failure exhibits many symptoms and signs. Fluid accumulates in the abdomen called ascites. It can become infected, spontaneous bacterial peritonitis (SBP). Jaundice is yellow color of the skin and eyes. The urine is dark and stools are light in color. Low testosterone causes sexual dysfunction in men. Ammonia levels are increased in the blood along with other neurotoxins. Forgetfulness, sleep disturbance and confusion are the result Bizarre behavior, DUI arrests and even coma can be experienced.

Lesions
Hemangiomas are benign common lesions in the liver that can be mistaken for liver cancer. They do not light up quickly with contrast and the contrast persists and puddles in hemagiomas. Hemangioms do not grow and if there is a question about an abnormality a repeat CT can be done 3-6 months later. Liver cancers get larger and hemangiomas do not grow.
